Pain ointment during pregnancy - effects

An analgesic ointment contains in its formulation a non-steroidal anti-inflammatory drug in the form of, for example, diclofenac or ibuprofen, or an opioid, such as tramadol ointment. The main difference between an ointment and an oral preparation is the local effect. When the medication is applied to the painful area, it is absorbed through the skin and has a local analgesic and anti-inflammatory effect. The mechanism of action of the analgesic drug is specific to the preparation contained in the ointment. Gel preparations are mainly used for acute pain and not for chronic pain.

Pain ointment during pregnancy - is it safe?

Most of the preparations classified as non-steroidal anti-inflammatory drugs (NSAIDs) are not suitable for use during the first and third trimesters. In the first weeks of pregnancy, they significantly increase the risk of miscarriage as well as the occurrence of heart defects in the fetus. On the other hand, when non-steroidal anti-inflammatory drugs are used during the 3rd trimester of pregnancy, there is a risk of premature occlusion of the ductus arteriosus, commonly called the Botall duct, which ultimately leads to the development of primary pulmonary hypertension in the newborn, a very serious complication. The second trimester of pregnancy is the safest period for taking NSAIDs, but the benefits of their use must outweigh the potential adverse effects. For this reason, experts also recommend not taking topical anti-inflammatory and analgesic medications, which include substances such as ibuprofen, diclofenac, ketoprofen, or naproxen. This is due to the fact that no studies have yet been conducted on pregnant women and the possible effects of such preparations. However, it is suspected that despite their local effect, the drugs may enter the bloodstream and have a potentially harmful effect on the development of the fetus.

Safe ways to manage pain in pregnancy

The drug of choice for all types of pain during pregnancy is paracetamol. Since it has no effect on the developing fetus, it can be used throughout pregnancy. The single dose of paracetamol is 1 gram and can be repeated every six hours. Unfortunately, no ointments containing paracetamol are currently on the market, so the use of preparations in this form is contraindicated during pregnancy.

Safe methods of pain relief include non-pharmacological methods such as:

  • Massage
  • Kinesiotaping
  • Use of a pressure-relieving belt

In case of persistent back pain, it is advisable to consult a physiotherapist who will try to relieve the persistent pain with methods that are not dangerous for the pregnant woman.
